We are recruiting a Reablement Leader to join our compassionate team.

When joining our Community Reablement and Outreach team, you will work in providing an island-wide service to vulnerable adults in their own homes across the Island.

In return for a great career opportunity, you’ll experience job satisfaction, knowing that our service and the work you will do supports people who are moving home from hospital or who live at home and need extra support to regain or optimise their independence.

You will play a vital part in undertaking the following duties:

  • Undertaking initial assessments
  • Case co-ordinating people
  • Ensure our clients reablement journey is appropriate to meet their needs.
  • line manage a team of support workers.
  • Work in a fast-paced environment
  • We work in partnership with the Island’s hospital who complete initial assessments on ward rounds and identify goals to enable them to return home.
  • Gain strong professional relationships within the health and social care sector.
  • Full UK driving licence and own vehicle is essential to meet the travel requirements for this role. Business use car insurance will be a required as your personal vehicle will be used in connection with your employment, when a pool car is unavailable.

 

For you to bring out the best in this challenging but fulfilling role, having a flexible approach to roster patterns that includes weekends and bank holidays will be essential.

You will be at the forefront of service delivery, leading by example and ensuring high quality services for people using our provision and their families and/or carers.

PLEASE NOTE: Out of Hours Standby and Call Out

You will be required to take part in the Out of Hours Stand-by Duty Rota as and when requested by management.  Standby and call out duties are paid in accordance with the IWC Terms and Conditions 2012 document.
