This report contains the results from the service questionnaire that people complete once they have finished being supported by the Community Outreach and Reablement Service. This report looks at completed feedback forms completed in quarter 3 (Jul-Sep) and quarter 4 (Oct-Dec) of 2022 and comparing the data.
There is a statement from the Registered Manager on how the service intends to improve the service based on the results of the feedback received. Individuals are also invited when completing the questionnaire to add their own views and opinions of which a selection have been added on the last 2 pages of this report.
