SEN Documents

This page lists the documents that the Local Authority currently uses as part of the Education, Health and Care Needs assessment process. If you are already a part of the Education, Health and Care Needs assessment process you will be provided with the relevant documents as required throughout the process.

The documents listed below, are provided to make you aware of the documents involved in the process but also to feedback any comments/suggestions on the documents that we currently use. 

To provide feedback to us, please access the appropriate document, download a copy, print it off and make any changes to that document that you feel are necessary with an explanation of why the change will further enrich the form.  You can then forward this document on to us by using either the address found on the ‘contacts’ tab above or via email to sen@iow.gov.uk

This flowchart sets out the process for completing an Education, Health and Care Needs Assessement.  The assessment is set out in the SEN Code of Pratice (2014) and must be completed within 20 weeks.

This is the form that your child’s school/education setting will fill in when a new assessment has been started. This information will then be used by the SEN Assessment and Review Team when it writes a draft Education, Health and Care Plan for your child.

Our Story

 

This is the form that parents/carers use to give their views and family history as part of their child’s education, health and care needs assessment.  If parents/carers would like, they can obtain help in completing this form from someone such as their Casework Officer, a family friend or parental support service. The form can be typed on a computer or handwritten.

My Story - for Early Years and Primary pupils

 

This is the form a child/young person uses to give their views as part of the education, health and care needs assessment. The child/young person can complete the form on their own using words, pictures, drawings or photographs or, an adult or member of school staff can help them.

Although this form has been written for a child in an early years setting or primary school, older children can use it if they would like to.

EHC Reply Slip

This is the form that parents/carers fill in when they receive their child’s draft Education, Health and Care Plan.

By filling in this form, parents/carers can say whether they are happy with the draft plan, ask for a meeting with their Casework Officer to discuss the plan and name the school/education setting they would like their child to attend.

Parents/carers have 15 days (calendar days - including weekends) to contact the Local Authority when they receive the draft Education, Health and Care Plan
