Short Breaks Membership

Service Description: The Short Breaks team are pleased to introduce the Short Breaks membership card. This card will give each child or young person access to the Short Breaks activity provisions and facilities.

The Short Breaks membership card will reduce the amount of registration forms which are required to be completed by parents or carers each time thier child or young person participates in a new Short Breaks activity. Parents or Carers are required to completed one membership application form per child or young person with a disability and/or addition to access to the whole Short Breaks programme.
